The protocol outlines the personalised care processes within primary care for the first 12 months following a patient’s cancer diagnosis. Includes coding guidance.

A letter to send to patients within 3 months of diagnosis, inviting them for a cancer care discussion. This satisfies QOF CAN005. The letter has been uploaded to practice clinical systems (April 2024).
