The Personalised Care Programme is managed through a number of delivery groups responsible for specific projects.  Below is an overview of the delivery groups and key contacts.  

Personalised Care Delivery Group

This group is responsible for supporting the implementation of Personalised Care Core Interventions in to patient pathways.  These include Holistic Needs Assessments (HNAs), Personalised Care Support Plans (PCSPs), Cancer Care Reviews (CCRs) and Personalised Stratified Follow-up (PSFU).

This group also oversee The Alliance’s strategy for improving access to health and wellbeing resources for all patients diagnosed with cancer and the The Live Well with Cancer (LWwC) Programme.

Genomics Delivery Group

This group brings together all organisations across Greater Manchester who are responsible for embedding genomic testing in to oncology pathways.

Psychological Support and Mental Health Delivery Group

The group is made up of multi-disciplinary professionals and patient representatives from across the cancer system  to improve whole pathway for psychological care from diagnosis, through treatment, cancer rehabilitation, recovery, living with and beyond cancer and end of life care. This includes, but is not exclusively, psycho-oncology teams who share expertise across the whole pathway, including consultation and training with primary care and “Talking Therapies” services.

Late Effects Delivery Group

This group was established towards the end of 2024 and brings together clinicians across Greater Manchester with a passion for improving support for patients suffering with the late effects of cancer and its treatment.
